Job summary

Compass Health Network is seeking a Community Support Specialist to provide personalized, community-based support for adults with behavioral health and substance use recovery goals. You will meet clients where they are—at home, in the office, and in the community—helping them access housing, healthcare, and other vital services.

You will drive to client locations as part of service delivery and collaborate with an integrated care team to document progress and outcomes.

Qualifications

• Must meet CSS requirements listed in the posting.
• Valid driver's license and ability to transport clients.
• Experience with adults in behavioral health preferred.

Responsibilities

• Support clients in reaching personal recovery and wellness goals.
• Provide coaching and education on life skills, health, and community resources.
• Help clients access housing, healthcare, employment, and other essential services.
• Work closely with healthcare providers, families, and community partners.
• Assist with appointments and provide transportation as clinically needed.
• Maintain accurate, timely documentation.
