Job summary

Compass Health Network seeks a Community Support Specialist to partner with adults in behavioral health and substance use recovery goals. You will meet clients where they are—at home, in the office, and in the community—providing coaching, life skills, and access to essential services.

The role involves driving to client locations, documenting progress, and collaborating with healthcare providers and families to support recovery in a compassionate, team-based setting.

Qualifications

• High School/GED required and human services education preferred.
• Four years of qualifying experience or related degree/experience mix.
• Must include delivery of services to individuals with mental illness, substance use disorders, or developmental disabilities.

Responsibilities

• Support clients in reaching recovery and wellness goals.
• Provide coaching on life skills, health, and community resources.
• Help clients access housing, healthcare, employment and essential services.
• Work with healthcare providers, families, and community partners.
• Assist with appointments and transport as needed.
• Maintain accurate, timely documentation.
