Job summary

RIM Recruitment Professionals is assisting a California-based healthcare and social services organization in hiring a Community Health Worker. The role provides person-centered support to Medi-Cal members in Enhanced Care Management and Community Supports, including outreach, housing navigation, and resource connection across the service area.

Two years of relevant experience are expected; a Bachelor's degree is preferred, and CHW certification is welcome but not mandatory.

Qualifications

• At least two years of professional experience in community health, outreach, care coordination, or related field.
• Experience enrolling members into managed health plan programs is required.
• Bachelor’s degree in Social Work, Psychology, Sociology, or related discipline preferred.
• Community Health Worker certification preferred but not required.
• Knowledge of Medi Cal, Medicare, CalAIM, Enhanced Care Management, or Community Supports preferred.
• Strong communication, advocacy, and documentation skills.

Responsibilities

• Conduct outreach and educate individuals about available healthcare and social service programs.
• Engage members and assist with enrollment into managed health plan programs.
• Manage caseload and coordinate referrals from health plans.
• Help members navigate healthcare, housing, behavioral health, and social service systems.
• Conduct home and community visits to assess needs and provide health education.
• Connect members with medical, housing, behavioral health, and community resources.
• Advocate for individuals with complex physical, behavioral, or social challenges.
• Use motivational interviewing techniques to encourage positive changes and self-advocacy.
• Collaborate with healthcare providers, social workers, health plans, and community organizations.
• Maintain accurate, timely, confidential documentation of all interactions.
• Collect and report required program and operational data.
