Job summary

Molina Healthcare is seeking a Community Health Worker to support community-based member advocacy and navigation of health plan benefits. You will meet members in homes, shelters, and provider offices, providing non-clinical support and resource guidance to improve care management.

Requires at least 1 year of health care experience, CHW certification in some states, and a valid driver’s license for travel. Strong communication and Microsoft Office skills are essential.

Qualifications

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required for certain states.
• Valid driver's license and reliable transportation for travel.
• Ability to multitask while speaking with members.
• Excellent customer service and organizational skills.
• Willingness to learn other programs and software systems.
• Proficiency in Microsoft Office.

Responsibilities

• Engages with members as an advocate and resource to support management of health care needs.
• Collaborates with the health care services team via non-clinical duties in the field.
• Empowers members by helping navigate and maximize health plan benefits.
• Assists members in accessing social services and community resources.
• Provides outreach to locate and support disconnected members with special needs.
• Conducts research to locate members not reached through normal channels.
• Participates in ongoing or project-based outreach efforts.
• Guides members to maintain Medicaid eligibility and other resources.
