Ideal candidates have health care experience with underserved populations, CHW certification (depending on state), and strong communication skills. Travel up to 80% locally is required, with a remote field-work setup.

Qualifications

• 1+ year health care experience with underserved or special needs populations.
• CHW certification may be required for some states.
• Valid driver's license and reliable transportation for field travel.
• Ability to multi-task while speaking with members.
• Excellent customer service and communication skills.
• Strong organizational and time-management abilities.
• Willingness to learn new programs and software systems.

Responsibilities

• Engage with members as an advocate and resource for health care needs.
• Provide non-clinical paraprofessional support in the field (home visits, shelters, provider offices).
• Assist members in navigating Medicaid benefits and scheduling appointments.
• Help connect members with social services and community resources.
• Conduct outreach to locate disconnected members needing support.
• Travel 50-80% locally as required by state/contract.
