Qualifications

• High school diploma or equivalent; associate or bachelor's degree preferred.
• 2+ years in community health, social services, or related field.
• Excellent interpersonal, communication, and engagement skills.
• Proficiency with EHR and basic computer systems; bilingual abilities preferred.

Responsibilities

• Spend ~65% of time in the field (homes, sites, events).
• Travel within assigned county for member and partner meetings.
• Provide health education, care navigation, and resource coordination.
• Document services in real-time and support transitions of care.
