Job summary

UPMC Health Plan in Butler County is seeking Service Coordinators to join the Community HealthChoices team. This hybrid role involves daily community travel to assess, plan, and connect Members with medical, behavioral, and social services, helping them stay safely at home.

Qualifications

• Bachelor's degree in social work, psychology, or related field (preferred)
• 3+ years of experience in social service/healthcare (required if no degree)
• Willingness to work in the field 75% of the time and maintain HIPAA confidentiality

Responsibilities

• Establish and build relationships with team members and providers to coordinate resources
• Manage an active caseload per state ratios and track participant progress for OPS reporting
• Prepare reports and update service plans within required timeframes
• Lead PCSP process and assist Members in obtaining LTSS services
• Coordinate access to LTSS, Medical Assistance, and non-Medicaid services for Members
• Inform Members about LTSS options and assist with fair hearing requests when needed
• Gather Member preferences, goals, and information to develop PCSP and reevaluate care levels
• Ensure LTSS eligibility activities and coordinate with providers and payer sources
• Coordinate with field-based and virtual teams to ensure compliant, confidential care
