Job summary

• BA/BS degree and at least 1 year of experience with the target population.
• Residence in North Carolina or willingness to relocate to NC.
• Experience in community-based or child welfare programs is preferred.

Responsibilities

• Provide integrated care management under CFSP, coordinating across health domains.
• Collaborate with County Child Welfare Workers to align health care with permanency goals.
• Conduct assessments (phone or in person) to identify member needs and plan care.
• Coordinate with multidisciplinary teams to support chronic conditions and needs.
• Set short- and long-term goals with members and caregivers.
