Job summary

Brandywine Counseling & Community Services, Inc. is seeking a Case Manager to support clients with substance use and mental health needs through assessment, care planning and coordination.

This is a field-based role with a focus on individualized care and service linkage. You will conduct Well Being Care Management assessments, maintain EMR updates, and ensure 75% of client contacts are face-to-face while coordinating with the TCM and PSS teams.

Qualifications

• Bachelor's degree in behavioral health or related field.
• 1 year experience in the Behavioral Health field.
• 1 year Case Management experience.
• Must have a valid driver's license with 2 points or less.

Responsibilities

• Participate in assessment, evaluation, planning, linkages, monitoring, advocacy and treatment of assigned clients.
• Provide services to clients dealing with substance abuse and/or mental health issues.
• Coordinate and monitor services with TCM & PSS teams.
• Complete Well Being Care Management assessments to inform an Individualized Care Plan.
• Maintain face-to-face contact with clients at least 75% of the time.
• Aid linkage of services needed by clients.
• Monitor services linked to clients continuously.
• Update the EMR regularly.
• Provide transportation for clients and assist referrals to transportation agencies.
• Deliver high-quality, appropriate services and uphold professional standards.
• Work primarily in the community outside of an office setting, up to 100%.

Skills

1 year behavioral health experience

1 year case management experience

Valid driver's license (2 points max)

Education

Bachelor's degree in behavioral health or related field
