Job summary

Merakey seeks a community-based Case Manager in Colmar, PA, serving Montgomery County. You will travel to clients' homes and community locations, building relationships and coordinating services to support recovery goals.

The role emphasizes hands-on, field-based work with mental health clients, including assessments, service planning, and collaboration with providers to improve community stability.

Qualifications

• Valid driver's license with clean motor vehicle record.
• Basic computer skills and experience with an EMR system preferred.
• Case Management experience in mental health or behavioral health is preferred.

Responsibilities

• Link consumers with resources and guide advocacy for self‑advocacy.
• Complete assessments, service plans, and determine level of care.
• Coordinate services with other providers and document requirements.
• Travel to homes and community locations to meet client needs.
• Maintain a caseload of 25–30 clients and meet with them per schedule.
• Attend monthly supervision and weekly team meetings.
