Job summary

Merakey is seeking a community-based Case Manager to serve clients in Colmar, PA, with duties spanning assessment, service planning, and coordination of care. You’ll work in the field, visiting clients in their homes and community settings, helping them achieve recovery goals in collaboration with Montgomery County resources.

The role requires managing a caseload of 25–30, completing required paperwork, and traveling across the county.

Qualifications

• Valid driver's license with clean motor vehicle report.
• Basic computer skills and experience with an EMR system.
• Case Management experience in mental health or behavioral health is preferred.
• Educational requirements include a Bachelor's degree or nursing or related combinations as stated.

Responsibilities

• Link consumers with resources and encourage self-advocacy.
• Complete assessments, service plans, and determine level of care.
• Complete paperwork from OMHSAS, Magellan, Montgomery County, and Merakey.
• Make home, community, and hospital visits; travel within Montgomery County.
• Maintain caseload of 25–30 clients and meet with clients per matrix schedule.
• Attend supervision monthly and team meetings weekly.
