Job summary

UnitedHealth Group in Evansville, IN is seeking an Integrated Care Coordinator to serve as primary care manager for older adults with complex medical or behavioral health needs. You will coordinate medical, social, and behavioral supports, develop person-centered care plans, and collaborate with internal teams and community partners to improve outcomes.

The role requires travel up to 75% within Indiana, telecommuting optional, and a schedule of Mon–Fri 8:00 am–5:00 pm.

Qualifications

• Master's or Bachelor's degree in social work, psychology, special education, counseling, or a related field.
• 2+ years of experience in long-term support services or working with older adults.
• 1+ years of experience with local behavioral health providers and community support organizations addressing SDoH.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Ability to travel up to 75% within the assigned territory to meet with members.
• Reside in Indiana and have access to reliable transportation & a valid US driver's license.

Responsibilities

• Serve as primary care manager and service coordinator for high medical risks with comorbid behavioral health needs.
• Engage members face‑to‑face and/or telephonically to complete comprehensive needs assessments.
• Develop and implement individualized, person-centered care plans with goals and interventions.
• Partner and collaborate with internal care teams, providers and community resources.
• Provide referrals and linkage to appropriate internal or community services.
