Job summary

UnitedHealth Group is seeking an Integrated Care Coordinator in Indiana to manage a panel of high‑risk older adults, coordinating medical and behavioral health care with attention to social determinants of health. You will assess needs in person or by phone and develop individualized care plans to improve health outcomes.

The role requires substantial travel across Delaware and Madison County and surrounding counties, with telecommute options where feasible.

Qualifications

  • Must have social work degree with 1+ years of case management with older adults.
  • 2+ years of experience in long-term support services or with older adults.
  • 1+ years of experience with local behavioral health providers addressing SDoH.
  • 1+ years of MS Office experience (Word, Excel, Outlook).
  • Ability to travel up to 75% within Indiana.
  • Residence in Indiana and access to reliable transportation.

Responsibilities

  • Serve as primary care manager and service coordinator for high medical risk members with behavioral health needs.
  • Conduct comprehensive needs assessments across medical, behavioral, functional, cultural, and socioeconomic domains.
  • Develop and implement person-centered care plans with clear goals and interventions.
  • Collaborate with internal care teams, providers, and community resources to execute the plan.
  • Provide referrals to Housing Navigator, Pharmacy Team, Peer Specialist, or other providers as appropriate.

Job description

The Integrated Care Coordinator will be the primary care manager for a panel of older adult members with a variety of medical and behavioral health needs. The role focuses on supporting members’ medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improve quality of care.

Work Schedule

Monday through Friday 8:00 am to 5:00 pm

Location

Delaware and Madison County, Indiana and surrounding areas. Local travel up to 75% and mileage is reimbursed at the current government rate. If you live within Indiana and can travel to Delaware and Madison County and surrounding counties, you will have flexibility to telecommute*.

Primary Responsibilities
  • Serve as primary care manager and service coordinator for high medical risk members with comorbid behavioral health needs.
  • Engage members face‑to‑face and/or telephonically to complete comprehensive needs assessments, including assessment of medical, behavioral, functional, cultural, and socioeconomic (SDoH) needs.
  • Develop and implement individualized, person‑centered care plans and service plans inclusive of goals, opportunities, and interventions/services aligned with the member’s readiness to change to support the best health and quality of life outcomes.
  • Partner and collaborate with internal care team, providers, and community resources/partners to implement the care plan.
  • Provide referrals and linkage as appropriate and accepted by members (may include internal consulting opportunities such as Housing Navigator, Pharmacy Team, Peer Specialist, etc. or community‑based provider referrals such as PCP, specialists, medication‑assisted therapy referrals, etc.).
Required Qualifications
  • Must possess one of the following:
    • Master’s Degree in Social Work
    • Bachelor’s Degree in Social Work, Psychology, Special Education, Counseling, or a related degree with at least 1+ years of case management experience with older adults, individuals with disabilities or those with serious mental illness.
  • 2+ years of experience in long‑term support services or working with older adults.
  • 1+ years of experience with local behavioral health providers and community support organizations addressing SDoH (e.g., food banks, non-emergent transportation, utility assistance, housing/rapid re-housing assistance, etc.).
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook.
  • Ability to travel up to 75% of the time within the assigned territory to meet with members and providers.
  • Reside in Indiana.
  • Access to reliable transportation and possess a valid U.S. driver’s license.
Preferred Qualifications
  • Demonstrated experience/additional training or certifications in Motivational Interviewing, Stages of Change, Trauma‑Informed Care, Person‑Centered Care.
  • Experience working in team‑based care.
  • Experience in serving individuals with co-occurring disorders (both mental health and substance use disorders).
  • Background in Managed Care.
  • Bilingual in Spanish or other language specific to market populations.
Physical Requirements
  • Ability to transition from office to field locations multiple times per day.
  • Ability to navigate multiple locations/terrains to visit employees, members, and/or providers.
  • Ability to remain stationary for long periods of time to complete computer or tablet work duties.
Compensation and Benefits

The hourly pay for this role will range from $29 to $52 per hour based on full‑time employment. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements). We comply with all minimum wage laws as applicable.
