Job summary

UCHealth Poudre Valley Hospital in Fort Collins, CO is seeking a Co-Responder Program clinician to investigate, assess and plan interventions for patients facing social, emotional, and environmental challenges. This on-site role supports first responders with field crisis responses and coordinates community services.

Applicants must have a master's in social work or behavioral health, and hold an LCSW/LPC/MFT/ACD license with BLS within 90 days.

Qualifications

• Master's degree in Social Service or Behavioral Health related field required.
• Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Marriage and Family Counselor (MFT) or Licensed Addiction Counselor (ACD).
• BLS within 90 days of hire.

Responsibilities

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines prioritizes and provides and/or arranges for needed internal and external services/interventions. Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor and evaluate compliance with patient's treatment plan.
• Responds in the field as a co-responder with local law enforcement to patients in active crisis. Identifies appropriate resources including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the field response.
• Supervises and/or trains new staff, students and interns.
• Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision.
