Job summary

BayCare Health Systems is seeking an experienced Case Manager to join our team in Pasco County. This field-based role travels across Pasco County and supports adults or children with behavioral health needs, substance use disorders, or child welfare involvement.

The Case Manager will engage and assess, plan, coordinate, and monitor services, while handling documentation and professional responsibilities. Travel is required, with possible urine drug screening and on-call rotations depending on

Qualifications

• Bachelor’s degree in a related field (Social Work, Counseling, Psychology, Criminal Justice, Health Education, Special Education or other Human Services related field).
• BLS (Basic Life Support) certification and Driver's License.
• CBHCM (Certified Behavioral Health Case Manager) certification must be obtained within one year of employment (preferred for all case managers).

Responsibilities

• Provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, or involvement in child welfare.
• Demonstrate competency in Engagement & Assessment; Service Planning & Delivery; Coordination, Linkage & Monitoring; Documentation; and Professional Responsibility.
• Coordinate service delivery and provide up-to-date referral resources to ensure continuity of care.
• Maintain and manage caseload based on needs, strengths, barriers and preferences of the individuals served.
• Travel to the person served and conduct urine drug screening or transport clients as required by program.
• May be required to participate in a 24-hour on-call rotation and perform other duties as assigned.
