Job summary

BayCare Health Systems is seeking an experienced Case Manager to join our compassionate team in Pasco County. This field-based role serves adults and children with serious mental health conditions and/or substance use disorders, across BayCare Behavioral Health programs.

The Case Manager coordinates services, conducts assessments, and links clients to community resources while traveling throughout Pasco County.

Qualifications

• Bachelor’s degree in Social Work or related Human Services field.
• 1 year full-time experience with behavioral health populations (adults or children).
• BLS certification and Driver’s License; CBHCM within one year is preferred.

Responsibilities

• Provide direct behavioral case management to adults or children with serious mental health conditions, SUD, or child welfare involvement.
• Coordinate service delivery and provide up-to-date referral resources for continuity of care.
• Maintain assigned caseload and tailor services to individual needs and preferences.
• Understand and apply service delivery regulations and district policy.
• Perform services at locations convenient to clients; travel is required.
• May conduct urine drug screening and transport clients; some programs require 24-hour on-call rotation.

BayCare Health Systems is currently seeking an experienced Case Manager to join our outstanding and compassionate team in Pasco County! This is a field-based position that works throughout all of Pasco County.

• Location : BayCare Behavioral Health- Child Treatment Center, Pasco County
• Status: Full-Time, Hourly
• Shift Hours: Vary
• Shift: Days
• Weekend Work : Occasional

The Case Manager Is Responsible For

• Case Managers provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, and/or those whose involvement in the child welfare system requires.
• The Case Manager should demonstrate competency in the domains of Engagement and Assessment; Service Planning and Delivery; Coordination, Linkage and Monitoring; Documentation; and Professional Responsibility.
• The Case Manager coordinates the service delivery for individuals, and provides community partners with up to date available referral resources to provide continuity of care.
• Case Managers effectively maintain and manage their assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
• The Case Manager possess a clear understanding of the service delivery system in accordance with regulations and district policy.
• Case managers provide services at the time and location that is most appropriate to the person served. Travel is required.
• Case Managers may also be required to conduct urine drug screening and transport clients, depending on the program.
• Some programs may also require a 24 hour on-call rotation. Case Managers will perform other duties as assigned.

Required

Education:

• Bachelor’s degree related to related fields, Social Work, Counseling, Psychology Criminal Justice, Health Education, Special Education OR Other Human Services Related Field.

Experience

• For targeted case managers working with children have 1 year full-time experience working with children with behavioral health concerns.
• For targeted case managers working with adults have 1 year full time experience working with adults with behavioral health concerns.
• Preferred Experience : 1 year of full-time experience with the identified population

Certification/License

• BLS (Basic Life Support)
• Driver's License
• F or targeted case managers (CBHCM) Certified Behavioral Health Case Manager certification must be obtained through the Florida Certification Board (FCB) within one year of employment.

Preferred

for all case managers.

• Targeted Case Managers required to bill Medicaid.
