Job summary

BayCare Health Systems is seeking an experienced Case Manager to join the compassionate team in Hernando County. This field-based role serves Pasco County, traveling to clients in community settings.

You will coordinate services for adults and children with behavioral health concerns, perform assessments, and ensure continuity of care with community partners. Travel and on-call duties may be required, with some programs needing urine drug screening.

Qualifications

• Bachelor’s degree related to Social Work, Counseling, Psychology or other Human Services field.
• BLS and Driver's License required.
• Targeted Case Manager certification (CBHCM) must be obtained within one year of employment.

Responsibilities

• Provide direct behavioral case management services to adults or children with serious mental health conditions or substance use disorders.
• Coordinate service delivery and refer to up-to-date community resources for continuity of care.
• Maintain caseload and travel to meet clients as needed; may conduct urine drug screens and transport clients.
• May require 24-hour on-call rotation; perform additional duties as assigned.
