US Fertility is seeking a Fertility Nurse Case Manager to join the Milwaukee, Wisconsin team. The role coordinates patient care, monitors treatment plans, and collaborates with physicians and staff to support patients through fertility journeys.

Qualifications

• RN license required and in-state (no limitations).
• BSN preferred.
• Fertility/IVF experience or women's health background preferred.
• Proficient phlebotomy skills (venipuncture and specimen handling).
• Ability to manage a caseload and coordinate care.
• Computer proficiency and EMR documentation.
• Strong interpersonal skills and teamwork.

Responsibilities

• Coordinate fertility patient care and case management.
• Monitor patients through treatment plans with education and guidance.
• Communicate orders and lab results with interpretation.
• Provide compassionate support to patients and families.
