Job summary

Gurwin Healthcare System in Commack, NY seeks a Registered Nurse for our Licensed Home Care Agency (LHCSA) to visit patients in their homes and supervise certified Home Health Aides on the East End.

Responsibilities include traveling to patient homes, conducting assessments, supervising aides, and documenting visits. Competitive per diem opportunities available with a dynamic work environment.

Qualifications

$90 for Initial visits $65 for revisits and $50 for discharge visits

EDUCATION AND EXPERIENCE REQUIREMENTS

A current New York State Nursing License.

A Baccalaureate Degree in nursing is preferred.

Previous home care experience is required.

Comfortable with laptop/tablet and integrating them into a home visit.

PHYSICAL WORK REQUIREMENTS:

Ability to travel to patients' homes to make home visits.

Ability to make clinical and managerial decisions within the scope of professional nursing practice and to perform clinical procedures completely and independently.

RESPONSIBILITIES INCLUDE, BUT NOT LIMITED TO:

Driving to visit patients in their homes.

Assessments and continuing care visits to patients who are admitted to the long-term care LHCDA program.

Supervision of home health aides including instructions for implementing written plans of care.

Notifying the agency staff of any change in the patient's condition.

Documentation of visits.

Other nursing services as needed.
