Job summary

Gurwin Home Care Agency in Commack, NY, is seeking a Registered Nurse to visit patients at home and supervise certified Home Health Aides for the East End LHCSA. A New York State Nursing License is required; a Baccalaureate Degree in nursing is preferred.

Responsibilities

Driving to visit patients in their homes.
Assessments and continuing care visits to patients who are admitted to the long-term care LHCDA program.
Supervision of home health aides including instructions for implementing written plans of care.
Notifying the agency staff of any change in the patient\'s condition.
Documentation of visits.
Other nursing services as needed.
