Four Oaks is seeking a compassionate Solution Focused Meeting Facilitator to bring families and support teams together in Cedar Rapids, IA. You will guide conversations, help identify actionable plans, and promote stability and long-term success for children, youth, and families.

The role emphasizes organizing meetings, documenting progress, and coordinating with social services staff while training in SBC, SFM facilitation, and YTDM as part of ongoing professional growth.

Qualifications

• Be at least 21 years old with a high school diploma or GED.
• Bachelor’s or Associate’s degree in a related field is preferred and helpful.

Responsibilities

• Coordinate and facilitate Solution Focused Meetings (SFMs) and Youth Transition Decision Making (YTDM) meetings.
• Create a welcoming, respectful environment for families and youth to share goals and concerns.
• Guide productive discussions to identify challenges and develop actionable plans.
• Prepare families and team members before meetings to ensure readiness and participation.
• Collaborate with HHS workers, FCS providers, and other professionals for coordinated support.
• Identify community resources and services to promote stability and self-sufficiency.
• Maintain accurate documentation and ensure services meet agency requirements.
• Complete required SBC, SFM facilitation, and YTDM trainings.
