Qualificações

• Bachelor's degree or PhD in Pharmacy is required.
• RPH license/certification from Texas is required.
• 2+ years hospital staff pharmacist experience preferred.
• 20 hours of sterile compounding training accredited by ACPE or ASHP is required.

Responsabilidades

• Promote pharmaceutical care by managing the medication process and preventing drug-related problems.
• Resolve actual drug-related problems and ensure patient safety.
• Educate patients, caregivers, and the community on medications.
• Conduct discharge medication reviews and counsel patients/caregivers on therapy.
• Maintain investigational drug records per departmental policy.
• Contribute to the medication management process.
• Comply with respiratory fit testing requirements for safety.

Formação académica

Bachelor's degree or Ph.D. in Pharmacy

RPH certification from the state of Texas

2+ years hospital staff pharmacist experience

20 hours of training in compounding sterile products accredited by the Accreditation Council for Pharmacy Education or the American Society of Health-System Pharmacists
