Qualifications

  • Master’s degree and license in Social Work (LMSW, LCSW preferred) or Mental Health Counseling (LMHC).
  • 5 years of work experience in a child welfare setting without a license.
  • 18 months of experience with children and families, preferably in a child welfare setting.

ABOUT THE JOB

The Family Team Conference (FTC) Coordinator schedules and tracks required conferences for programs and facilitates conferences that are not facilitated by ACS. The purpose is to assist the team in making quality decisions related to child safety, well-being, and permanency. Additionally, the facilitator provides support to program staff and families. FTC Coordinators work with multiple borough-based programs.

KEY ESSENTIAL FUNCTIONS:

  • Engage participants in non-ACS facilitated Family Team Conferences.
  • Facilitate conferences to help participants reach consensus.
  • Participate in debriefing meetings immediately following FTCs.
  • Ensure all conference participants sign the Summary and Action Plan following conferences.
  • Coordinate with program staff and families for scheduling and rescheduling FTCs as needed.
  • Schedule separate conferences for suspected or known DV batterers or victims.
  • Identify families with limited proficiency and make arrangements for Language Interpretation Services if necessary.
  • Maintain a tracking log for requested and completed FTC facilitations.
  • Float within the designated borough to provide FTC facilitation services to multiple programs.
  • Conduct FTCs according to ACS protocol and FTC model.
  • Review family history in CONNECTIONS and hard case records prior to the FTC.
  • Consult with Case Planners/Therapists about unresolved questions or concerns after case review of documentation prior to the FTC. Provide in-home and office FTC facilitation as necessary.
  • Review and identify external child welfare resources and community linkages.
  • Use conferences to model effective strategies for engagement, assessment, and problem-solving when appropriate.
  • Attend agency and ACS trainings and meetings.
  • Use knowledge of different types of FTCs and EBMs (e.g., high, medium, low) to make referral recommendations.
  • Work collaboratively with team members on challenging issues to resolve barriers or develop effective interventions.
  • Utilize knowledge about ACS standards and protocol to help the team arrive at safety, well-being, and permanency decisions.
  • Other duties as assigned.

EDUCATIONAL / TRAINING REQUIRED: Master’s degree and license in Social Work (LMSW, LCSW preferred) or Mental Health Counseling (LMHC) with at least 18 months of experience with children and families, preferably in a child welfare setting.

Master’s degree without a license with 5 years of work experience in a child welfare setting.

-OR-

Bachelor’s degree with 5 years of work experience in a child welfare setting.

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE:

  • Bilingual/Bicultural Spanish. Knowledge of motivational interviewing, family systems theory, and/or trauma-informed care. Good strengths-based engagement and facilitation skills, as well as strong organizational skills.
  • Prior experience working with children and families using an evidence-based model.
