Northern Rivers Family of Services is seeking a Family Support Worker to join the Short-term Services Program in Schenectady, NY. You will provide intensive case management and help families resolve immediate needs within 90 days of admission, coordinating with DSS and community partners.

You will work onsite with occasional travel up to 50%, maintain confidential records, build trusting relationships with families, and ensure timely referrals and supportive services across office, home, school,

• Requires an Associate’s degree and 1–2 years of related experience.
• Must have a valid NYS driver’s license.

• Model and demonstrate problem-solving and decision-making skills.
• Work with SCDSS case managers to identify service needs of at-risk families.
• Have thorough knowledge of community services in Schenectady County.
• Help families obtain services by making referrals, providing transportation, accompanying them to appointments, and assisting with applications.
• Maintain good working relationships with DSS/Support Unit caseworkers.
• Complete progress notes or other case records promptly.
• Maintain cultural sensitivity, competence and client confidentiality.
• Other duties as assigned.

As a Family Support Worker, you will work as part of the Short-term Services Program which was developed as an intensive case-management service to rapidly resolve the concrete and referral needs of families entering the Child Welfare System. By successfully resolving these issues within 90 days of admission, families are at a reduced risk for being referred to Long-term CPS. Services are provided in the office, home, school, and community. You will report to the Coordinator of Short-term Services and will work onsite with community-based travel required.
