Role Description
This is a full-time on-site role for a Family Support Worker/Home Visitor located in the New York City Metropolitan Area. The Family Support Worker will be responsible for providing support services to families, conducting case management, and working with children to ensure their well-being. Daily tasks involve engaging with families, coordinating with various service providers, and documenting all interactions and services provided.
The Home Visitor is responsible for delivering high-quality, comprehensive early childhood and family development services to pregnant women, infants, and toddlers (birth to age 3) in their homes. The Home Visitor supports school readiness by promoting positive parent-child interactions, healthy development, and family well-being. This role requires cultural sensitivity and commitment to serving diverse families, including those with children discharged from the NICU or experiencing high-risk circumstances.
Key Responsibilities:
• Conduct weekly 90-minute home visits for each assigned family (12 families per caseload).
• Implement a research-based home visiting curriculum to support children’s development in all domains.
• Partner with parents to set individualized goals for their child’s development and family needs.
• Facilitate group socialization experiences twice per month to build peer interactions and parent connections.
• Monitor and document child progress using observation tools and screenings (e.g., ASQ-3, ASQ-SE, DRDP, ELAP).
• Provide prenatal education and postnatal support to expectant families.
• Connect families to health, nutrition, mental health, and community resources as needed.
• Collaborate with medical providers, especially NICU discharge teams, to ensure continuity of care.
• Maintain accurate and timely documentation, case notes, and reports in compliance with Head Start Performance Standards and agency policies.
• Participate in regular reflective supervision, team meetings, and ongoing professional development.
Qualifications
• Associate’s or Bachelor’s degree in Early Childhood Education, Social Work, Family Development, or a related field required.
• Infant/Toddler CDA with experience will be considered if pursuing a relevant degree.
• At least 1-2 years of experience working with young children (0-3) and families in a home-based or community setting.
• Experience supporting families in underserved communities, including those with complex medical needs (e.g., NICU) is preferred.
• Strong communication, organization, and relationship-building skills.
• Knowledge of child development, trauma-informed care, and family engagement practices.
• Clearance of background checks and health screenings as required by federal and state regulations.
