Damar Services is seeking dedicated individuals to join as Habilitation Services Providers, serving youth and caregivers under Indiana Medicaid Waiver programs. This independent contractor role focuses on developing social, emotional, and decision-making skills to improve functioning and quality of life for children aged 7–17 and their families.

Qualifications

• Must hold current certification to provide Habilitation and Family Support Training services through Indiana Medicaid Waiver programs.
• High school diploma or GED required.
• Must be at least 21 years old.
• Two years of qualifying experience working with or caring for children and youth with SED, as defined by the DMHA.
• Valid Indiana driver's license.
• No felony convictions or other disqualifying criminal offenses.

Responsibilities

• Provide Habilitation Services and Family Support Training to clients.
• Train and psychoeducate in self-direction to help clients develop self-advocacy, civil rights, and community involvement.
• Help clients acquire self-help, social, and adaptive life skills; promote self-control and responsibility in support.
• Assist clients in meeting treatment outcomes.
• Provide guided supervision, monitoring, psychoeducation, mentoring or support to help the client acquire and retain skills; include psycho-educational activities and community-based opportunities.
• Complete required documentation in a timely manner.

Damar Services is seeking dedicated individuals who are passionate about working with youth and their caregivers to join our team as Habilitation Services Providers. Clients include children between 7-17 years of age and caregivers for waiver services that are non-DCS cases.

The goal of the Habilitation service is to enhance the participant's level of functioning, quality of life and use of social skills, as well as build the participant's and family's strengths, resilience and positive outcomes. The Habilitation service provider helps the participant accomplish these skills through development of social, emotional, and decision-making skills.

This opportunity offers a flexible schedule covering Marion County and surrounding counties!

Essential Duties and Responsibilities

Services May Include

• Habilitation Services
• • Training and psychoeducation in self-direction designed to help clients develop self-advocacy skills, exercise civil rights, and/or become more independent and involved in the community (such as learning time management skills)
  • • Acquiring and maintaining self-help, social, and adaptive life skills
    • Acquiring skills to encourage self-control and responsibility regarding support received and needed
• Assist clients in meeting treatment outcomes
• Guided supervision, monitoring, psychoeducation, demonstration, mentoring, or support to assist the participant with the acquisition and retention of skills
  • Psycho-educational activities, hobbies (to promote positive connections in the community in which the client lives), unpaid work experiences (such as volunteer opportunities), and assisting in developing support systems for both the client and their family)
• Complete required documentation in a timely manner
