Job summary

Damar Services is seeking dedicated individuals to join our team as Habilitation Services Providers. You will work with youth aged 7–17 and their caregivers under waiver services.

The role focuses on building social, emotional, and decision‑making skills to improve everyday functioning and resilient outcomes. As an Independent Contractor covering Marion County and surrounding areas, you will deliver structured activities, complete required documentation promptly, and collaborate with families

Qualifications

• Current certification to provide Habilitation and Family Support Training (FST) services through Indiana Medicaid Waiver programs is required
• High school diploma or GED required
• Must be at least 21 years of age
• Two years of qualifying experience working with or caring for children and youth with SED as defined by DMHA
• Valid Indiana driver's license
• No felony convictions or other disqualifying criminal offences

Habilitation Services Providers - Independent Contractor

Damar Services is seeking dedicated individuals who are passionate about working with youth and their caregivers to join our team as Habilitation Services Providers. Clients include children between 7-17 years of age and caregivers for waiver services that are non-DCS cases.

The goal of the Habilitation service is to enhance the participant’s level of functioning, quality of life and use of social skills, as well as build the participant’s and family’s strengths, resilience and positive outcomes. The Habilitation service provider helps the participant accomplish these skills through development of social, emotional, and decision‑making skills.

This opportunity offers a flexible schedule covering Marion County and surrounding counties!

Essential Duties and Responsibilities

Services May Include

• Habilitation Services
• • Training and psychoeducation in self‑direction designed to help clients develop self‑advocacy skills, exercise civil rights, and/or become more independent and involved in the community (such as learning time management skills)
    • Acquiring and maintaining self‑help, social, and adaptive life skills
    • Acquiring skills to encourage self‑control and responsibility regarding support received and needed
• Assist clients in meeting treatment outcomes
• Guided supervision, monitoring, psychoeducation, demonstration, mentoring, or support to assist the participant with the acquisition and retention of skills
  • Psycho‑educational activities, hobbies (to promote positive connections in the community in which the client lives), unpaid work experiences (such as volunteer opportunities), and assisting in developing support systems for both the client and their family
• Complete required documentation in a timely manner

Knowledge, Skills and Abilities
