Job summary

SHIELDS for Families in Lynwood, CA is seeking a dedicated Family Support Specialist to deliver home-based services under the Healthy Families America program. The role supports high-risk families, builds parenting skills, and promotes strong family functioning through personalized Family Service Plans and ongoing case coordination.

This full-time, 8:30 am to 5:00 pm position is in-person with office and field components.

Qualifications

• Bachelor's degree in Social Work or related field.
• Bilingual skills preferred.
• Valid California driver’s license and reliable transportation.

Responsibilities

• Develop and monitor Family Service Plans (FSPs) and conduct required assessments.
• Provide home-based services and crisis intervention as needed.
• Coordinate referrals and services with partners; conduct follow-up and evaluation.
• Maintain accurate documentation in compliance with program standards and funder requirements.
• Participate in team meetings, case conferences, and required trainings.
