Job summary

Rise Inc Family seeks a Family Support Specialist to coordinate in-home services and support families under ADCS contracts. The role requires engaging with diverse families, maintaining accurate documentation, and meeting timelines.

Work is remote with occasional in-office tasks from our Maricopa, AZ office, and may involve evenings, weekends, and extensive local travel to serve Marin County clients.

Qualifications

• Bachelor level degree or higher required.
• Excellent written and verbal communication; able to document findings clearly.
• Ability to travel within the county/metro area and work flexible hours including evenings/weekends.

Responsibilities

• Provide short-term, time-limited coordination of services to families per contract requirements.
• Meet with families regularly to address needs and resolve concerns using evidence-based resources.
• Accept responsibility for cases assigned by Team Lead and support family outcomes.
• Ensure timely completion of weekly progress notes and tasks as contracted.
• Be flexible to meet client schedules, including evenings and weekends; travel extensively as needed.

This is a remote position with occasional in office work out of our Maricopa, AZ office, This position delivers the Family Connections and Nurturing Parenting Programs using evidence-based models, in-home case management and supportive services to families based on the needs, concerns, and/or stressors identified by Arizona Department of Child Services (ADCS) and the Family in order to enhance protective capacities and factors within the home to stabilize and preserve the home environment through behavioral changes by the parent/caregiver through the implementation of the RISE Services Inc. mission, vision, and values and compliance with all company and state regulations and policies.Job Responsibilities:Provide short term, time-limited coordination of services and resources to all assigned families based on the needs, concerns and stressors of the child(ren) and the family, and enhance the protective capacities or strengthen the protective factors identified and outlined by ADCSMeet with families based on contract requirements and their needs (weekly, monthly) to address and resolve issues or concerns, using evidence-based curriculum and community resources including but not limited to: nutrition and home management, communication and negotiation skills, child development, education on safe sleep, systems of supportAccepts responsibility for cases assigned by the Team Lead. Works collaboratively with the Team Lead to support and assist the family in reaching positive outcomesEnsure contract timelines are met including weekly progress notes, and all other assigned tasksBe flexible to meet the needs of the families served, including evenings and weekends; accepts emergency phone responsibility when assigned.
