Job summary

Communitas, Inc. in Wakefield, MA is seeking a Family Support Coordinator to empower families through home-based services and DDS/DCF collaboration.

You will translate family needs into community supports, access entitlements, and coordinate with state agencies to improve outcomes. The role offers full-time hours, a competitive hourly pay of $24.00–$25.50 depending on Bachelor's or Master's level, plus benefits and tuition reimbursement.

Qualifications

• Bachelor's Degree required in social work, education, human services or related field.
• Up to two years of family support case management experience preferred.
• Must pass company background and reference checks.
• Valid Driver’s license, reliable transportation, and acceptable driving record.

Responsibilities

• Inform the family and assist them in accessing supports and services within the community.
• Assist families in obtaining all state and federal entitlement benefits such as SSI, CommonHealth clinical/medical supports, and behavioral supports.
• Collaborate with other agencies and supervise support staff in families' homes.
• Develop and implement Individualized Family Treatment Plans and document progress for reviews.
