Family Support Coordinator

The CBHC Family Support Coordinator (FSC) is an integral member of the interdisciplinary Youth/Young Adult Team, providing care coordination and case management support on behalf of youth in partnership with caregivers. The FSC assists families with navigating youth-serving systems, connection and referral to services, and family support and advocacy. Care coordination in this role is provided through the lens of lived experience as a caregiver to youth living with behavioral health challenges.

In this vital role, the FSC will:

  • Assist parents in their development as advocates for their children, participating in activities that are designed to address Service plan health related social needs.

    • Attend school or community meetings alongside parents/caregivers as needed.
    • Provide aid to parents/caregivers in accessing appropriate outside resources.
  • Provide care coordination on behalf of youth with caregivers, navigating child serving systems like DMH, DCF, schools/IEP services, and CBHI services.

  • Develop recommendations on appropriate community resources and supports which would benefit the family with both the family and interdisciplinary team.

  • Complete all documentation as required by the program, funding sources and regulatory agencies in a complete, accurate and timely manner and in compliance with agency and relevant state protocols.

Schedule: Full-Time, 40 hours

  • Weekend work is not required, but highly monetarily incentivized with a $5 weekend differential.

Pay: $22.27/hourly

  • Additional $1 for CPS
  • Additional $2 for CARC

Required Skills

  • Good verbal and written communication skills required.
  • Ability to work independently as well as part of a collaborative team.
  • Valid driver’s license and access to a reliable vehicle required.

Required Experience

  • Must have lived experience as a caregiver of a youth with behavioral health challenges.
  • A minimum of 1 year of work experience as a family partner or very closely related role required
  • Personal lived experience with behavioral health challenges (substance misuse or mental health) strongly preferred
  • Bachelor’s degree in human services field preferred but not required
  • Must have experience in navigating any of the child and family-serving systems (especially CBHI services) and teaching family members who are involved with the child and family-serving systems.
  • Must be able to establish a rapport and work effectively with a diverse, multi-cultural group of parents, children, family members and other program staff members.
  • Understanding of child development and family issues that impact upon child/parent relationships required.
