Network for Hope is seeking a dedicated Family Support Coordinator to guide families through organ, tissue, and eye donation with empathy and clarity. You will collaborate with hospital teams to assess eligibility, coordinate conversations, and ensure families receive continuous support from intake to post-recovery follow-up.

The role emphasizes compassionate communication, education for families, and liaison work between families and medical staff, reflecting NFH’s mission to save and enhance

Qualifications

  • Experience in marketing, communication, teaching, health care, counseling, or public service-related roles.
  • Bachelor’s degree in allied health, education, business, pastoral care, psychology or related field preferred; extensive experience may substitute.

Responsibilities

  • Respond to hospital referrals within designated timelines and communicate with AOC.
  • Lead empathetic discussions with families, tailoring approach to each situation.
  • Conduct Donor Risk Assessment Interviews (DRAIs) with family members.
  • Provide continuous emotional support and educate families about the donation process.
  • Follow up with families after the recovery process to inform them of outcomes.
  • Educate hospital staff about donation procedures and contribute to policy development.

Job description

As a Family Support Coordinator (FSC), you will play an essential role in guiding families through the delicate process of organ, tissue, and eye donation. Through your support, you will be instrumental in helping families make informed decisions, offering compassion and understanding every step of the way.

Key Responsibilities
Referral Management
  • Respond independently to hospital referrals within designated timelines, ensuring timely communication.
  • Report relevant patient information to the Administrator on Call (AOC) for further coordination.
  • Confirm donor eligibility in collaboration with the AOC.
  • Collaborate with the healthcare team to assess the readiness of the legal next of kin or family for donation conversations.
Donation Conversation
  • Lead empathetic, compassionate discussions with families experiencing sudden loss, offering a safe space for grief and emotions.
  • Build rapport with the healthcare team to gather necessary information about family dynamics, determining the best time for donation discussions.
  • Advocate for donation with sensitivity, tailoring your approach to each family's unique circumstances.
  • Assist families in understanding the grave prognosis.
Donor Family Management
  • Conduct uniform Donor Risk Assessment Interviews (DRAIs) with family members to assess suitability for donation.
  • Provide families with accurate, timely information regarding the donation process, collaborating with medical and hospital staff as needed.
  • Act as a liaison between the family and the healthcare team, ensuring their needs are met and supporting their wishes.
  • Follow up with families after the recovery process to inform them about donation outcomes.
Family Support
  • Provide continuous emotional support to families throughout the donation process.
  • Educate families about the donation process, helping them understand each step clearly and compassionately.
  • Support families and help them prepare for their departure from the hospital.
  • Offer memory-making services and connect families to community resources through NFH-led initiatives.
  • Maintain regular communication with NFH staff, ensuring coordinated and consistent support throughout the process.
Documentation
  • Obtain and document authorization for donation.
  • Maintain accurate, detailed records of family interactions, dynamics, and healthcare team communications.
  • Follow established documentation protocols to ensure accurate, thorough case details.
Other Duties
  • Educate hospital staff about the authorization, care process, and donation procedures. You may assist with presentations and training sessions for hospital partners.
  • Contribute to the development of department policies and improve procedures based on hands-on experience and feedback.
  • Participate in case reviews, meetings, and continuous learning opportunities to stay current on best practices.
  • Other duties as assigned by the Family Support Manager.
Qualifications
Education:

College degree (Associate or Bachelor's level) in allied health, education, business, sales or marketing, pastoral care, psychology or related field required. Extensive experience and healthcare certifications may substitute for education requirements and will be reviewed on a case-by-case basis.

Experience:

Experience in marketing, communication, teaching, health care, counseling, pastoral career other public service-related required. Hospice care, bereavement counseling, pastoral counseling, social work, organ or tissue donation and other helping professions are examples of the training and experience relevant to this position.
