Qualifications

  • Bachelor's degree in a related field (preferred) or 2-3 years of extensive case management experience.
  • 2+ years human services experience, preferably with developmental disabilities.
  • Excellent communication and organizational skills.
  • Proficiency in MS Office; ability to learn new programs.
  • Valid driver's license, auto insurance, reliable transportation.
  • Ability to lift up to 20 pounds and perform related physical activities.

Responsabilités

  • Support families per NH regulations and waivers.
  • Attend IEP meetings and collaborate with educational and community teams.
  • Assist with transition planning from school to adult supports, including natural supports and provider selection.
  • Coordinate assessments (Support Intensity Scale, HRST) and ensure timely referrals.
  • Help individuals maintain public benefits and navigate legal processes (Guardianship, Power of Attorney).
  • Develop, implement, and monitor Person-Centered Service Plans and Agreements.
  • Maintain contact with families and professionals, documenting all interactions.

Manager's Note:
