Job summary

Responsibilities

• Attend CORE Training and related trainings.
• Counsel parents during assessments on topics like Safe Sleep and PPD.
• Make independent decisions for client welfare.
• Provide community referrals and trainings on Healthy Families.
• Screen and assess new families in the program.
• Refer families to community resources as needed.
• Transport families to Healthy Families services when necessary.
• Document screens in the HFI database weekly.
