Job summary

Florida Department of Health in Pinellas County seeks a Human Services Counselor I to establish trusting relationships with diverse families and manage care coordination in the community. The role supports fathers and families by linking to community services and documenting progress in HMS.

The counselor will use trauma-informed practices, promote positive parenting, and collaborate with partners to meet program goals while maintaining accurate records and participating in team supervision.

Qualifications

• At least one year of experience working with children and/or families
• At least one year of experience working with high-risk populations
• Reliable personal transportation daily and a valid driver’s license
• Willing to work a flexible schedule including evenings and weekends

Responsibilities

• Establishes trusting relationships with families and provides case management in the community.
• Links fathers to supports and coordinates services available in the community.
• Documents case progress and maintains electronic health records for each client served.
• Promotes father involvement and participates in community partnerships.
• Performs assessments and reviews service activity to meet program requirements.
