Job summary

Homes for the Homeless in Queens, NY, seeks a dedicated Case Manager for our Jamaica facility to support families by coordinating services, maintaining records, and helping secure housing and resources.

The role requires a Bachelor’s or Associate’s degree, familiarity with case management, and proficiency in Outlook, Word, and Excel. Bilingual skills (Spanish or another language) are a plus, with a strong benefits package and supportive work environment.

Qualifications

• Bachelor’s or Associate's Degree.
• Familiarity with case management preferred.
• Knowledge of Outlook, Word, and Excel.
• Bilingual in Spanish or other language is a plus.

Responsibilities

• Maintains and updates digital and physical records of all families as required by governing agencies.
• Collaborate with clients to develop service plans based on an assessment of each family’s needs.
• Assist families with referrals to outside agencies for employment, education, parenting, legal, medical, and mental health services.
• Maintain and update case files bi-weekly to document progress.
• Assist families with moving into permanent housing.
• Ensure that clients maintain an open public assistance case.
• Adhere to all city and state guidelines for maintaining documents.
• Conducts unit inspections.
• Perform other duties as assigned.
