Children’s Wisconsin is seeking an integrated health navigator to improve health outcomes by linking clinical care with community resources.
The role supports underserved families, identifies needs (food, housing, childcare), and coordinates referrals within the care team, using the EHR for documentation.

As an integrated member of the healthcare team, the goal is to improve health outcomes by serving as the liaison between the clinical setting and the community. This role empowers underserved patients and families by identifying and utilizing resources to support achievement of whole child and family health goals, working towards long term sustainability and independence. Serves as a facilitator among patient families and internal & external resources and demonstrates strong relationship building and problem-solving skills.
High School graduate or Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED) required Associate's Degree in human services, public health, or related field preferred
2+ years working with underserved/vulnerable population required Experience working with families of diverse ethnic, cultural and socioeconomic backgrounds required Experience navigating EHR/Epic preferred
