Role and Responsibilities

• Outpatient primary care for seniors with 3-5 complex diseases (e.g., heart disease, diabetes, hypertension)
• Manage a panel of up to 650 patients, seeing 10-12 patients per day
• Work Monday-Friday, 8 am-5 pm with a call schedule based on provider numbers
• Collaborate with a dedicated care team including MAs, schedulers, and referral specialists
• Participate in a value-based care model, starting with a new center opening in May 2025

Requirements
