Position Details

1. Full Time
2. Hours: Monday - Friday 8 am - 5 pm
3. Employment Type: Employed
4. Average Patients Seen: 10-12 patients per day
5. Sign-On Bonus available
6. Compensation: Base salary plus performance-based bonuses, with earning potential up to $300,000. Provider incentive bonus is performance-based, not RBU or fee-for-service.
