Conviva Care Center , a subsidiary of Humana Inc. , is seeking a full-time Primary Care Physicians (Family, Internal, or Geriatric Medicine) for our clinics in our Corpus Christi, Texas market .

You have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinates of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a multidisciplinary approach.

A Primary Care Physician opportunity on our team can offer:

• Monday-Friday 8:00am-5:00pm schedule and minimal call or Saturday coverage

• More time with your patients - typical one-hour-long initial patient appointments - typical 40- to 45-minute-long exam time

Required Qualifications:

• Doctor of Medicine or Osteopathic Medicine

• Board certified or Board eligible in Family Medicine or Internal Medicine

• Valid, unrestricted license in the state of TX

• Current, unrestricted DEA license

Preferred Qualifications:

• Minimum of two to five years directly applicable experience

• Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment

• Knowledge of Medicare guidelines and coverage

• Knowledge of HEDIS measures and quality indicators
