Job Description:

Colorado Physician Partners, PLLC is seeking a Family Medicine physician to join our Lakewood, Colorado clinic. This clinic is one of 8 in an independent private practice network across the Denver area.

Our practice is part of the independent practice association (IPA), PHPprime, which provides services such as care coordination, practice transformation coaching, risk adjustment coding education, population health management, data analytics, specialist network management, and large group contracting.

About our clinics:

• Independent outpatient private practices in the Denver area
• The team includes Providers, support staff, a social worker, and an RN/patient educator who serve as care coordinators. Mental health support is available at most practices.
