Job summary

Qualifications

• Licensure: State of New Mexico or Compact State Nursing License.
• Degree from accredited nursing program with BSN completion within 5 years if conditional.
• Minimum ADN preparation with expectation of BSN completion within 5 years of hire.
• BLS required.

Responsibilities

• Responsible for walk‑in and telephone triage.
• Coordinates patient care through collaboration with the patient, family, and healthcare team.
• Provides patient care by assessing, planning, implementing, evaluating, and documenting individual patient needs, actions taken, and patient responses.
• Responsible for case management based on PCP panels, in collaboration with network providers, including nurse visits and telephone management.
• Delegates or assigns aspects of patient care to healthcare team members according to their validated competency.
• Completes documentation within scope of practice.
• Performs other functions as required.
