Qualifications

• 5+ years of related experience preferred.
• Knowledge of public health principles and practices preferred.

Responsibilities

• Deliver primary care services to individuals and families across all age groups.
• Collaborate with Behavioral Health and other providers to encourage integrated health.
• Provide direct patient care including assessments and patient management plans.
