Requirements include an MD or DO, Wisconsin license, completion of a Family Medicine residency, board certification or eligibility, DEA and BLS, and at least 5 years of practice.

We are seeking a Physician to provide high quality, patient-centered care to patients of all ages, from birth through older adulthood. The Physician will independently assess, diagnose, and treat patients within their scope of practice while collaborating with interdisciplinary services and other care team support staff as needed.
