Job summary

Pillar Health, a Division of Kenosha Community Health Center (KCHC), seeks a Family Practice Nurse Practitioner to deliver high-quality, patient-centered care in southeastern Wisconsin. You will independently assess, diagnose, and treat across the lifespan, collaborating with a multidisciplinary team.

Qualifications

  • Graduate of an accredited Nurse Practitioner Program.
  • Current Wisconsin Advanced Practice Registered Nurse License.
  • Current national board certification as a Family Nurse Practitioner (FNP) or other certification demonstrating qualification to provide care across the lifespan.
  • Minimum of 5 years of clinical experience in family practice or primary care settings.
  • Current DEA registration for prescribing controlled substances.
  • Current BLS certification.
  • Ability to provide care to patients of all ages, from newborns to elderly adults.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal skills.
  • Commitment to providing compassionate, patient-centered care.

  • Provide comprehensive preventative, acute, and chronic care to patients across the lifespan, beginning at birth.
  • Perform patient assessments, physical examinations, and medical history reviews.
  • Diagnose and manage acute and chronic health conditions.
  • Order, perform, and interpret appropriate diagnostic tests and imaging.
  • Prescribe medications, treatments, immunizations, and other therapies as appropriate.
  • Develop and implement individualized treatment and follow-up plans.
  • Educate patients and families regarding medications, treatment options, disease prevention, healthy lifestyles, and management of chronic conditions.
  • Maintain accurate and timely documentation in the electronic health record.
  • Collaborate with physicians and interdisciplinary care team members in the management of complex patients.
  • Participate in quality improvement efforts and support organizational standards for patient care, productivity, compliance and service.

Pillar Health, A Division of KCHC, is growing and looking to add an experienced Family Practice Nurse Practitioner to serve its patients in southeastern Wisconsin.

Position Summary

We are seeking a Nurse Practitioner to provide high quality, patient-centered care to patients of all ages, from birth through older adulthood. The Nurse Practitioner will independently assess, diagnose, and treat patients within their scope of practice while collaborating with interdisciplinary services and other care team support staff as needed.
