Geriatrics Directorship - West Springfield, MA
Opportunity for a Board Certified Geriatric Medicine, Internal Medicine, or Family Medicine physician to join its Program of All-Inclusive Care for the Elderly (PACE) organization.
Position Details
- Full-time, permanent
- Serves as Outpatient PCP and skilled facility provider
- Schedule: Monday to Friday, 8:00 am - 4:30 pm. Approximately seven (7) on-call days (evenings/weekends) with weekend telephone triage.
- Patient load varies from 2-8 patients per day
- Leadership role requiring skills in strategy, vision, resource alignment, organizational growth, and change management.
- Establish and maintain a plan to achieve operational goals as part of the leadership team.
- Foster collaborative relationships with managers, peers, direct reports, and patients by promoting teamwork, consensus, and conflict resolution.
- The physician provides primary medical care, including assessment, care planning with the interdisciplinary team, direct patient care, and evaluation of care effectiveness. Functions collegially with other healthcare professionals, making independent decisions regarding medical needs and health regimens, and performing indicated medical procedures.
- Must have a valid driver’s license, proof of insurance, and reliable transportation.

Requirements
- Board Certified in Internal Medicine or Family Medicine (required)
- Board Certified in Geriatric Medicine (preferred)
- Minimum 1 year experience working with frail or elderly populations
