Job description

Our client has an excellent full-time opportunity for a Board Certified Geriatric Medicine, Internal Medicine, or Family Medicine physician to join its Program of All-Inclusive Care for the Elderly (PACE) organization located in West Springfield, Massachusetts. This is an excellent opportunity for an experienced physician to join an exceptional PACE program!

Job Description:

  1. Position serves as Outpatient PCP and skilled facility
  2. The schedule for this full-time opportunity is Monday to Friday, 8:00 am - 4:30 pm. On average, there are seven (7) on-call days (evenings/weekends) with weekend telephone triage.
  3. Number of patients seen per day varies from 2-8
  4. Understand, incorporate, and demonstrate the Mission, Vision, Values, and Guiding Behaviors of Trinity Health and Trinity Health PACE (TH PACE) in behaviors, practices, and decisions.
  5. Consistently demonstrate TH PACE Values to all internal and external customers, creating a warm, calm environment throughout the office.
  6. Execute assignments in a collaborative culture across all divisions within TH PACE.
  7. Reflect leadership skills by defining and sharing a strategy and vision, aligning resources, growing and sustaining the organization, and embracing change.
  8. Establish and maintain a plan with the leadership team to achieve operational goals.
  9. Foster collaborative relationships with managers, peers, direct reports, and customers by building consensus and resolving conflicts.
  10. Provide primary medical care to participants, including assessment, developing a care plan with the interdisciplinary team, delivering direct care, and evaluating the plan's effectiveness.
  11. Make independent medical decisions, perform procedures as indicated, and work collegially with other healthcare professionals.
  12. Must have a valid driver’s license, proof of insurance, and transportation.
  13. Use EMR system: TruChart.

Compensation and Benefits:

  • Pay range: $208,000 - $241,000 (depending on experience)

Submission Requirements:

  • Board Certified in Internal Medicine or Family Medicine - REQUIRED
  • Board Certified in Geriatric Medicine - PREFERRED
  • MA license (or willingness to obtain) - REQUIRED
  • CDS Certification & DEA Registration (prior to start) - REQUIRED
  • Minimum 1 year experience working with frail or elderly populations - REQUIRED
  • BLS certification (prior to start) - REQUIRED
  • Clean malpractice record - HIGHLY PREFERRED
  • Experience with EMR TruChart - HIGHLY PREFERRED
