Our client has an excellent full-time opportunity for a Board Certified Geriatric Medicine, Internal Medicine, or Family Medicine physician to join its Program of All-Inclusive Care for the Elderly (PACE) organization located in West Springfield, Massachusetts. Job Description:
- Position serves as Outpatient PCP and skilled facility
- The schedule for this full-time opportunity is Monday to Friday, 8:00 am - 4:30 pm. On average, there are seven (7) on-call days (evenings/weekends) with weekend telephone triage.
- Number of patients seen per day varies from 2-8
- Understand, incorporate, and demonstrate the Mission, Vision, Values, and Guiding Behaviors of Trinity Health and Trinity Health PACE (TH PACE) in behaviors, practices, and decisions.
- Consistently demonstrate TH PACE Values to all internal and external customers, creating a warm, calm environment throughout the office.
- Execute assignments in a collaborative culture across all divisions within TH PACE.
- Reflect leadership skills by defining and sharing a strategy and vision, aligning resources, growing and sustaining the organization, and embracing change.
- Establish and maintain a plan with the leadership team to achieve operational goals.
- Foster collaborative relationships with managers, peers, direct reports, and customers by building consensus and resolving conflicts.
- Provide primary medical care to participants, including assessment, developing a care plan with the interdisciplinary team, delivering direct care, and evaluating the plan's effectiveness.
- Make independent medical decisions, perform procedures as indicated, and work collegially with other healthcare professionals.
- Must have a valid driver’s license, proof of insurance, and transportation.
- Use EMR system: TruChart.
Compensation and Benefits:
- Pay range: $208,000 - $241,000 (depending on experience)

Submission Requirements:
- Board Certified in Internal Medicine or Family Medicine - REQUIRED
- Board Certified in Geriatric Medicine - PREFERRED
- MA license (or willingness to obtain) - REQUIRED
- CDS Certification & DEA Registration (prior to start) - REQUIRED
- Minimum 1 year experience working with frail or elderly populations - REQUIRED
- BLS certification (prior to start) - REQUIRED
- Clean malpractice record - HIGHLY PREFERRED
- Experience with EMR TruChart - HIGHLY PREFERRED
