Job description

Position Summary: Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.

Essential Duties and Responsibilities:

1. Provide psychiatric medication services in accordance with Eventus WholeHealth site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine related to long-term care facilities.
2. Review and sign MARs as requested by facilities, including all orders specific for Primary Care, within 24 hours if faxed or emailed, such as phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders, and other relevant orders.
3. Review and address labs within 24 hours of receipt.
4. Manage narcotic prescriptions for facility patients according to Eventus WholeHealth Narcotic Rx Policy.
5. Provide facilities with adequate time during visits for administrative paperwork.
6. Complete documentation and submit billing records as per policies, and provide a hard copy of documentation within 48 hours for the patient’s medical record.
7. Establish and maintain open communication with facility staff and administration, providing verbal or written feedback on medications and other pertinent information.
8. Meet with the facility care coordinator or administrator before starting and after completing patient visits each day.
9. Adhere to Eventus WholeHealth standards, policies, procedures, and all legal, regulatory, and ethical guidelines.
10. Maintain patient confidentiality at all times, complying with HIPAA regulations.
11. Consult with the clinical supervisor before refusing any resident, family, or facility request regarding patient care.

Qualifications/Education:

• Masters degree in Nursing from an accredited institution and current, valid license to practice medicine issued by the state medical board. All necessary certifications must be current.
• OR graduated from an accredited physician assistant program, passed the national certification exam, and holds a state license as a Physician Assistant. Geriatric, family, or psychiatry specialization is preferred.
