DayToDay Health is seeking full-time physicians to join a thriving primary care practice focused on elderly patient care across multiple locations in the United States. Responsibilities include caring for homebound, long-term care, and hospital patients, with a practice mix of roughly 75% Long Term Care/SNF and 25% hospital work, adjustable by preference.

The team includes Primary Care/Geriatrics physicians, Physician Assistants, and Nurse Practitioners sharing responsibilities such as night
