Geriatrics Director - Mishawaka, IN
Located 5 miles south of South Bend, PACE Director
Position Details
- Full-time, permanent position
- Supervised by the Executive Director
- Provides medical direction and supervision for participant care
- Responsible for clinical outcomes, participant care delivery, and overseeing the QAPI program
- Develops and maintains medical policies, procedures, standards, and protocols
- Provides leadership to Primary Care Providers (PCPs) and APPs
- Guides program colleagues and local community providers
- Must have a driver’s license, proof of insurance, and transportation
- Proficient in Microsoft Suite (Word, Excel, PowerPoint) and familiar with email, calendaring, and other software
- Excellent organizational, customer service, flexibility, and problem-solving skills
- Willing to travel regionally as needed
- Conducts history and physical assessments, develops treatment plans, and provides necessary services across participant locations
- Participates in applicant interviews and assessments for PACE enrollment
- Discusses medical conditions and prognosis with participants and caregivers to support care planning
- Works collaboratively within a shared, team-oriented culture
- Contributes to the nursing treatment program
- Sets and works toward operational goals as part of leadership
- Participates actively in the interdisciplinary team, providing education and advice on diagnoses and treatment options
- Engages in team and family conferences and decision-making
- Educates participants and families on health maintenance and chronic conditions
- Supervises healthcare and treatment programs developed by the team
- Performs physical exams, orders and evaluates tests, and records results systematically
- Skilled in physical assessment and chronic disease management for frail older adults
- Desires to work in an innovative healthcare environment
- Adapts to changing priorities based on participant needs
- Participates in provider on-call rotation as required
Compensation and Benefits
Salary range: $231K - $280K, plus 10% bonus

Requirements
- Board-certified in FM/IM or Geriatrics Fellows
- Minimum 1 year experience working with elderly or frail populations (no new graduates)
